To verify your own benefits, you’ll need:
Step One: Locate your member services phone number on the back of your card
Step Two: Dial the phone number and navigate through the prompts to reach a representative
Step Three: Ask the representative – “I would like to request information about my in-network, outpatient mental health benefits, in office and through telehealth.”… Be sure to also ask about your deductible and if that impacts your copay.
*If they ask for CPT Codes, they are:
Outpatient mental health counseling – individual (90791, 90837)
Outpatient mental health – medication management (99205, 99214)
